Municipality of Westerkwartier: applying for an EU parking card for people with disabilities
Please note: You can only apply for an EU parking card for people with disabilities if you live in the Netherlands and are registered with a municipality.
Complete the form and include the attachments. You can request this form by telephone (14 0594) or pick it up at one of the counters of the town halls in Leek or Zuidhorn.
You can also submit a request by e-mail via the e-mail address backofficewmo@westerkwartier.nl. Please state your name, address, telephone number and the reason why you are applying in the e-mail.
- EU parking card for people with disabilities: € 18,15
- Medical assessment: € 53,00 regardless of whether the GPK is awarded
You pay a fee for this service. For more information, contact the municipality.
It will take 6 weeks to get your card. You must collect the card in person at the municipal offices.
