Apply for a certificate of life

There are 3 ways to apply:

  • Online with DigiD
  • In person at the front desk. You will receive the certificate immediately. Please make an appointment for this. You cannot come by without an appointment
  • Authorize someone. You can authorize someone to collect your certificate of life by completing, printing and signing the authorization form.

This can only be done in person at the front desk. You will be given the certificate immediately. You have to make an appointment for this. You cannot come by without an appointment.

  • Certificate of life: € 15.00 
  • Attestatie de Vita: € 15.70
