Applying for an Attestatie de vita (Certificate of life) in the Netherlands

The Attestatie de vita is a document that you can use outside the Netherlands to prove that you are alive. Some organizations, such as a pension fund, may request you to produce this document. In the Netherlands you can use the document Bewijs van in leven zijn (Proof of being alive) for the same purpose.

The Attestation de vita is a document in French and Dutch for international use.

Applying for an Attestatie de vita (Certificate of life) in the Netherlands

There are 2 types of proof of being alive:

  • Proof of being alive in the form of an extract from the Basic Registration of Persons (BRP). This is only for use in the Netherlands
  • The attestation de vita signed by an official of the registry office. This is for use abroad. For example, if you are entitled to a pension from abroad

A valid proof of identity (passport, identity card or driver's license)

If you have one: letter pension fund

Money: you pay the extract in cash or with pin

Has someone authorized you to apply for proof of life? Then you take with you:

The authorization form

Doctor's statement from the person who authorized you

Your own ID

A valid ID of the person who authorized you or a copy of it

Money: you pay the extract in cash or with pin

  • The Bewijs van in leven zijn (certificate of existence): € 9,20 
  • The Attestatie de vita (certificate of life) : € 16,60
