Applying for an Attestatie de vita (Certificate of life) in the Netherlands

The Attestatie de vita is a document that you can use outside the Netherlands to prove that you are alive. Some organizations, such as a pension fund, may request you to produce this document. In the Netherlands you can use the document Bewijs van in leven zijn (Proof of being alive) for the same purpose.

The Attestation de vita is a document in French and Dutch for international use.

Proof of life or certificate of life

You can apply for an Attestatie de vita  (certificate of life) from the municipality where you live and are registered. You can only make the application yourself. Someone else cannot do it for you. You must make an appointment with the municipality to apply for a certificate of life.

  • A valid identity document, such as your passport or driving licence.
  • Cash or a payment card. You pay the fee at the time of application.

Request a proof of life or certificate of life

How to request proof of being alive:

  • Log in with your DigiD at the municipality where you live.
  • Fill in the details.
  • You pay the extract online with iDEAL.
  • You will be sent home proof of being alive.

How to request a certificate of life:

  • You go personally to the municipality where you are staying.
  • You take with you:  your valid ID and possibly the message from the authority stating that you need proof of being alive
  • You pay the costs with the application. Please pay with card.
